Ensure that your data is PPR13 (Name, Surname, FULL ID numbers, contact details of the main member and beneficiaries) compliant to ensure it can be quoted without problems.

The information you provide to BDP will be kept confidential in line with our POPI policy. The data and information will only be shared with the administrator and insurer to provide you with a quote.

To enable us to provide you with a quote we will need the following information from you regarding your scheme:

• • Complete data with details of all insured policyholders and lives to be covered. The compliant data must contain the following information:
• o Full names and Surname of the policyholder.
• o Initial and Surname of the policyholders, spouse, children, and extended members.
• o Complete ID Numbers of all insured.
• o Cover / Benefit required for each policy.

• • Proof of underwriting where existing underwriting is in place.
• • Claims ratio from the responsible underwriter for 12 months.

BDP will provide you with a comparative quote within 7 days once the data was received. Should you accept the quote we will require the following information for onboarding the funeral scheme.

Complete PPR13 data.

• • Full names of policyholder, and Surname
• • Initials and Surname of Spouse, Children and extended family members.
• • Complete ID Numbers for all insured.
• • Contact details of each policyholder
• o Cellphone number
• o Physical address
• • Copy of CIPC document
• • Proof of address (not older than 3 months)
• • Certified copies (not older than 3 months) of all directors, members or where it is a friendly society the Chairperson, Treasurer and Secretary
• • For friendly societies and stokvels copy of the constitution
• • Proof of banking details (not older than 3 months) for the premium account.
• • Proof of banking details (not older than 3 months) for the business account.
• • BEE Affidavit (copy will be provided once quote is accepted)
• • Honesty and integrity declaration (will be provided once quote is accepted)
• • Proof of underwriting from the responsible Insurer
• • Claims ratio from the responsible insurer for 6 months.
• • Signed quote, accepting the conditions of cover.
• • FSP 5 Form for funeral parlours who are not FSP’s to be registered as a JR on the FSP.
• • Any additional information that may be required from the Insurer.

The waiting period will be waived for schemes that can proof existing underwriting for more than 6 months. The waiting period may be negotiated for self-underwritten schemes of more than 500 policyholders. The quotes are subjected to the due diligence and pricing provided by the Insurer in line with the risk profile of the funeral schem
